How to ask user to enter email address and name in google forms

HI,
I was trying to setup a google form which will basically have multiple choice questions.
I have selected the option where a user can submit the form only once.
But when i tested it i found that google forms do not record email ids etc.
I can see individual response but i cannot see who has submitted the response.

How do i automate google to send me the details of everyone who has submitted the forms ?
If its not possible is there any alternative ? I am thinking of creating a section in my form to specify name and email address of the user to ask them to enter it manually.

First of all, can people fill in the form WITHOUT logging in? If so, obviously, there's no automatic way to record the email.

Second, if they DO have to log in with their Google Account, are you using the paid Google Apps Domain to make this form?

If so, you can enable the option "automatically collect respondent's yourdomain.com username"
Obviously, that's probably not what you want (only collects your own colleague's usernames, not from the average google user).
If not on a Google Apps domain, then you have no choice at all (even though if you were, that options was too limited for you anyway).

So all in all, you'll probably end up using a name/email address field (which you cannot really verify, even if they needed to log in).
