It is possible that the orientation sensor has failed. You can try this app to see if you can force it into an orientation state.
https://play.google.com/store/apps/details?id=com.googlecode.eyesfree.setorientation&hl=en
You can also use a bubble level app to determine if your orientation sensor is indeed working. There are tons of those to choose from. Just search the Play Store for "bubble level"

If the above does not work try disabling some apps to see if that helps.

The two sensors for detecting rotation on Android OS are:

MAGNETIC and ACCELEROMETER sensors

http://stackoverflow.com/questions/6846743/which-sensor-for-rotating-android-phone

In your Phone app of your Samsung phone, dial *#0*# and press the dial key and it will launch the Test utilities.

http://www.askvg.com/hidden-secret-codes-for-samsung-galaxy-mobile-phones/

Select Sensor test and test on both MAGNETIC (self-test) and ACCELEROMETER (Image test) sensors and you will know whether it is a hardware problem.

Yes

It is broken from my experience.

Magnetic and Accelerometer are two different sensors.
