Windows 2012 Server USB drives not working

Windows 2012 R2 Standard server

I have three usb drives attached to this server.

Today after applying several windows updates restarted the server and I started getting event 51 all over the place along with several other messages.

I first noticed when I started up explorer you would see the drives appear and then disappear

So I removed all three then everything settled down

I started adding one at a time thinking one was bad.

So I placed the 3 TB USB drive on and it worked fine not blinking on and off.

Then I added one of the 2 TB usb drives and then it started blinking again.

I tried the other 2 TB drive and that caused the blinking too.

They are plugged into a usb hub

So I tried to plug them into the front usb port on the server (Dell PowerEdge 860 G2 8 GB Ram)

Same results they started blinking in explorer

I plugged both into a windows 7 pro laptop and they both are ok I can see folders and files no problem

What is up with Windows 2012 server?

I have other Windows 2012 servers that have usb drives but none have three I have only one other 2012 server that has two usb drives and it is ok

Any ideas?

Thanks

Tom

My thinking is that there may be a power issue.  Are these drives powered from the USB cable, or do they have individual power adapters?

In the USB 1.0 and 2.0 specs, a standard downstream port is capable of delivering up to 500mA (0.5A)

500 mA at 5V is only 2.5 watts and that is very little power to run a hard drive even with two-headed cables.

Given (a) the drives work on at least one other system, (b) the significant power pull of hard drives, and (c) the very limited amount of power that a USB port can deliver, I'd look further into power issues.  If the drives have power sockets, try attaching an AC adapter and see if the problem is alleviated.

(Running hard drives through a USB hub imo is asking for trouble, but that issue can be investigated separately.)

No they do not have a power adapter

I have another Windows. 2012 server that had two USB drives connect I added one that I had on the server with the problem and it works fine

Something on the server I think

I got from Staples a USB 3 Hub that is powered and I can plug 6 things into it. If you think the suggestion above about power has merit, get a USB 3 powered Hub and try it. It is probably worth your while to try this.

No. I just went into the store. Most hubs are USB-2 but they had a USB-3 hub and I picked it up.

Staple Part number was SPLS USB hub 7-port 718103257930.  One port is for a Micro USB plug and that is the cable that goes to the server.

That is why I walked in. The store had a USB-3 in stock. You can try a USB-2 as it should be fast enough.

Yes I see that

Just wondering why when I plug them directly into the usb port on the server they do not work correctly

why would a 3.0 hub make a difference?

As Dr. K pointed out, connecting the drive directly means it is getting all power from the USB port. Using a powered Hub will provide more power to the USB drive. This may help it work correctly .

Guys

Purchased a 4port 3.0 USB hub with power adapter and connected it to the server

the connected one of the usb drives that had the issue on this server and yes you guessed the exact same thing happened.

It can not be a power problem now

I have only one of the 3 usb drives connected currently as soon as I add the second one both start flickering in explorer

Event id 51 and 140 all over the place.

Any other ideas?

I don't really know at this point. Hopefully the USB Hub can be used somewhere else (we have them at clients).

How long is the USB cable to the drive? Can you try a shorter cable?

The cable are proprietary and about a foot long

I can only plug one in at a time now

I had all three connected before the last restart

This is not something new I am trying here

Has to be a solution somehow

Several of our 2012 and 2012r2 servers had a similar issues and it was Trend Micro antivirus that caused the issue..

I do not run Trend Micro

This is strange since it was working for a very long time and then it just stopped

Only errors/warning in event log are related to the device not connecting properly

Also strange is that I can only plug in one usb drive at a time now

Only one works

The server has 4 usb ports

two in the front that look like USB 2.0 only and 2 in the back not sure what they are.

In the back two I have a KVM usb cable which gives my Video KB and Mouse functions
a UPS USB connection  which is plugged into a 4 port usb hub and that is where I had all 3 usb drives connected.

Now only one usb drive is plugged into the hub now

I tried the drives in the front usb ports with no luck

Puzzeled.

So, you say it was working well before. What exactly did happen before the problem started occurring? Some Windows Update installed? Or some hardware added?
Have you checked the USB settings in BIOS?

I had two widows updates from wsus the same updates that were applied to my other windows servers.  No hardware updates at all The updates were windows security patches for windows 2012.

KB3179574 and KB3177723

I have another Windows 2012 server same Dell poweredge 860 with two usb drives and the same updates were applied on the same day I added a third usb drive to that system and had no problem at all

Again no hardware updates were applied.

What should I look for in the bios for usb?

Thoughts

Also not sure if the Dell PowerEdge 860 has USB 3.0 ports could not find any documentation on that either.

Uninstall the updates from this server and check if it works then better. As we cannot narrow the problem to its bottleneck in a different way - we need to exclude the possible causes one by one.

I would also boot the server from a WinPE CD and check if the USB drives are detected and working properly there. This would help to exclude the hardware problem.

Create an Installation Media for Windows Installation - usb or dvd. It will be the same as winpe. Plus as an EE expert you can get Paragon Software tools for free and create Recovery Cd based on WinPE using one of their Hard Disk Manager tools.

Waiting on them to approve my access.

Hopefully by the weekend so I can shutdown this server

I moved the usb drives to my NAS device and mapped them to this server but that causes many event 51 and event 3 during the backup process

I have another script that runs robocopy from one drive to another and that is failing also

I plugged a usb stick into the server tonight and got the same results they just blink on and off

The only usb drive that is still working also went crazy until I turned it off then back on

This is a strange one for sure

Will post after I do the test

Thanks

This update is not applicable to this system

This is a Windows 2012 R2 Standard 64 Bit

I solved this by doing the following

1. shutdown the server
2. unplug the power cord from the ups and let sit for 3 minutes
3. Power up server
4. went into device manager and uninstalled all the usb drive devices.
5. connected the usb hub
6. connected 1 of the usb drives waited for a short while
7. connected drive 2
8 connected drive 3

after a few uninstall of the devices every thing is now stabilized all drives connected to the original usb hub are fully functional.

Thank you every one for your help

Hi Thomas,

thanks for update. So the problem was indeed in Windows and the booting from boot CD would show it.
Good to hear that you resolved it.
BTW, for Windows 10 you need to use this ADK package: https://blogs.technet.microsoft.com/yongrhee/2015/08/11/download-windows-10-assessment-and-deployment-kit-adk/
Take care

Ok

Ran WINPE on server today.

All USB Drives appeared.
was even able to browse the folders and files on all the usb drives

Restarted server and no change

The usb drives continue to flash on and off  with Event 51 errors.

Since WINPE saw all the USB drives and yes they are plugged into a USB HUB there is nothing wrong with the hardware in my thoughts.

I even made sure vds service was started  no change.

I can plug the drives into any other windows 2012 or windows 2008 server and they work fine.

Even after deleting all the entries in device manager for the usb devices   no change

Any ideas

Something on the server I think

Something we have not mentioned here. Run SFC and maybe DISM

System File Checker:

Open cmd.exe with Run as Administrator and run SFC /SCANNOW.
Allow to complete, restart or, preferably, shut down the computer and then start up again.
Test to see if the problem has been corrected.

DISM:

Open cmd.exe with Run as Administrator.
DISM.exe /Online /Cleanup-Image /Scanhealth (takes 15 - 20 minutes).
DISM.exe /Online /Cleanup-Image /Restorehealth (takes 15 - 20 minutes).
Restart the computer and test.

You have done the most major repair short of reinstall (DISM) and that did not work. So beyond that there does not have to be a solution to the USB issue.

Now look at a workaround. Use a network drive instead of USB., That will work just as well. Then use the other server for attaching the USB drive if necessary

There is no such thing as "not an option" There is always a way. Do not use USB drives on this server.

have you considered partial hardware failure? maybe try installing PCIx or PCI USB card and test again. you could have overloaded the USB chipset having multiple drives connected to the same physical port on the server.

This could have been ask/answered already? Apologies..
How many physical ports does the server have? Have you tried connecting all your drives to physical ports instead of using the HUB..

This is a Dell PowerEdge 860 G2 1U server

It has 4 USB Ports 2 in the front and 2 in the back

Here is what is connected

all on the back two usb ports

KVM  for KB/Mouse and monitor one connection

USB HUB
       UPS
       3 TB USB Drive
       2 TB USB Drive
       2 TB USB Drive

I tried this before

KVM
USB HUB
3 TB USB Drive

On front two USB Ports
2 TB USB Drive
2 TB USB Drive

Both in a separate UB Port

Same results event 51 and flashing of usb drive in explorer.

I had Device Manager Open also to see what was happening

It looks like the devices are trying to re register  or re install

I really believe this is not a hard ware related issue but then again I am not an expert on this matter.

Can you recommend a pci card that will work on windows 2012? I can try that one with usb 3.0 ports
