I wear a lot of hats...

"The solutions and answers provided on Experts Exchange have been extremely helpful to me over the last few years. I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

You would have to put in text boxes where they are supposed to enter the information and then protect the document.

Edit to add: Also keep in mind that of you make a document an actual template format file, Word will use that to create a new document based on the template rather then open the template itself, which will help prevent the user fro accidentally writing over the template.

If you don't have to support pre-2007 Word users, you should use Content Controls as found in the Controls group of the Developer tab. For Word up to 2003, you will have to use Legacy Form fields hidden in the same group.

Protect the document for filling in forms via the Protect Document on the same tab
