I am not sure if reinstallation of weblogic server is not needed.

Maybe you can try searching for file basejpi.htm on your PC.

I have this file in 2 directories:
1. <<middleware_installation>>\Middleware\ORA_HOME\forms\templates\config
2. <<middleware_installation>>\Middleware\<<asinst_1>>\config\FormsComponent\forms\server

You can try to copy file from 1. to 2.

In such a case your (PC) Administrator can help you with permissions.

Was there any form maintenance or development that took place, which would explain a probable cause? I might run this by my SA, then my supervisor as a possible issue. Think, physical disk write fail.

I would recommend to solve this problem with yours administrators. I don't know how to re-create file basejpi.htm. Also, as David pointed - there can be disk write failings. We don't know if this is new error ...
