I wear a lot of hats...

"The solutions and answers provided on Experts Exchange have been extremely helpful to me over the last few years. I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

I know your requirement is to do this withOUT a third party tool however my answer to your question is that it is way too complicated to answer.  You are not the first to ask this question and to get this disappointing answer.

See here where the accepted answer also advises against and explains why doing it yourself is difficult.

Read all the responses here which also advise against it.

Same sought of answer.

I am sure you already know this but it appears as iTextSharp is the third party tool of choice and here is a tutorial on using it.
