Is it common in IT sections that when users/departments are requesting RFC (request for change), to use a standard form to get all the background info on the change. I have found that our section has a very fragmented approach to accepting RFC's, and there is no consistent/standard template form completed by the change requester, each seem to communicate the info to the IT section in different ways.<br />

I am trying to build a case for using a standrd RFC template, but some examples on risks in not using a standard form would help me build this case. Can anyone give any examples on risk of the lack of a RFC template form?

RFC forms change management

IT Administration is the processes and best practices for programming and development, and incorporates methodologies for managing activities and projects. Common methodologies include waterfall, prototyping, iterative and incremental development, spiral development, rapid application development, extreme programming and various types of agile methodology. The life-cycle "model" is a more general term for a category of methodologies, and a software development "process" a more specific term to refer to a specific process chosen by a specific organization.
