how does it happen with legitimate senders, I mean how it is malformed?

we have allowed malformed MIME but there is checkmark against mime fuzzy main header in our external smtp gateway and extract ole1.0 is also checked-don't know what it means

As I said the last time you asked this question, it could be malformed on purpose in some sort of hacking attempt, or just something happened to the message in transit. Or any other number of things mentioned above.
It is still safer to reject it. If the message is from a legitimate sender, they should get a notice back that their message wasn't delivered.

I am sorry I am not really sure what "mime fuzzy main header" means I can't help with that

in short it is the standard format for emails to be sent in to be recognized by other emailing systems (mail servers, anti-spam etc...)
a Malformed MIME is whenever an email is received and does not follow these standards in one or more way it could be treated as "unscannabled" by your mail anti-virus for example
