Online Forms

I need your suggestions about online forms which can help me to keep track of a progress activity.
In fact it should a simple page with names, text boxes and check boxes, more like a table.
I could use Google Spreadsheets, but I would prefer more like Google Forms.
Unfortunately the Google Forms behaves more like a survey, which all the time shows the same new interface/form like a reset. I would need to see more like a progress activity, meaning once a check box is set, then it should stay like that. The fact that is connected to a Google spreadsheet is good and I like it. I need to know who and when checked a specific check box.

But every time when is accessed it shows as a new survey and I need stay checked what was checked. Then near each check box a short text box where the people which will click the check boxes will put their name initials.
The form will be given only to the trust persons, but we are all over the world and we have to share such form.
So far I have only a Google Spreadsheet and anyone with a link may change its content, which is not very convenient.

What suggestions do you have?
Any chance with Google Forms/Spreadsheets or whatever other Google free service?
Any other free service/website with such forms?

I already have done before what you suggested, but I wanted something as a form.
I have and use Google Spreadsheet.
I tried Google Forms.
I tried some other online  forms.
For example this one:

Works fine if you do not click Submit
Then I asked technical support and they explained me why is hard to find such thing:

I guess my search stops here.
The only other way that I see is to make my form on my website.

It seems I have to stay with Google Spreadsheets for the moment as I started before I asked the question and later, when I have time to make a custom form on my own web-page somewhere.
