Are you logging into a domain on a network, or is this just at home?

When you log in the first time and it creates the profile, try logging off at least once so that it successfully saves everything, then log back in again.

Its a home laptop no domain the profiles are created properly logging in and out etc.. the fact that its still happening after a full reformat just boggles my mind
Try running this from a command prompt with admin rights - "sfc /scannow"

After that runs, do error checking on the hard drive. You'll have to reboot to fix that. Heck, do a full defragment as well.
