Is there a way to force Outlook 2016 to create a new profile in Online mode, without creating an OST file, but not with Cached mode disabled?
Thus leaving the "Use Cached Exchange Mode" box unchecked but not grayed out.

I do not think so. Cached mode CREATES an OST file so it can do its job.

I've discovered if you leave the settings for it not configured in the GPO it creates the profile in cached mode by default.
The only way I've been able to get it to not do so is to configure it to disabled. This prevents the user from being able to then check the box and start using cached mode at a later time.
What is needed is for it to be created in Online mode but with the user to have the ability to choose cached mode later.
