how can I update personal information at http://partnersforhealthtn.gov/?
Hello and Good Evening Everyone,
I am trying to help a friend update her personal information at http://partnersforhealthtn.gov/. For example, whenever she clicks on the link of Complete Your Biometric Screening on the right hand side of the homepage and enters her CareMark ID Number, a form in PDF format automatically opens identifying her full name, address, and phone number with clinical information to be entered by her doctor at the bottom of this form. Unfortunately, the telephone number automatically entered into this form is incorrect and can not be corrected on the screen because the form appears to be read only. Given this situation, does http://partnersforhealthtn.gov/ have an area for updating personal information so it will automatically appear corrected within Biometric Screening form after login with the CareMark ID number? There is another server, http://www.edison.tn.gov/, which also uses the CareMark ID number. Perhaps the personal information can be corrected through their website.

I believe the form is automatically generated either by partnersforhealthtn.gov or Edison.tn.gov. and independent of being generated by her personal physician. The form itself automatically has her name, address, and phone number inserted into the required fields. At the bottom of the screen, there are clinical measures such as blood pressure, glucose level, cholesterol level, and so forth which is left blank and to be printed out and taken to her doctor for completion.
At this point, I will give her the number mentioned within your feedback in addition to any other contact information I might be able to find too.
