I created a pdf fillable form from Word and uploaded it here: <a href="http://www.sonoma.edu/exed/intersession/summer-2017-exception-form" rel="ugc">http://www.sonoma.edu/exed/intersession/summer-2017-exception-form</a>. The first field, i.e., the name is not editable. However, when I tested it on my local machine, it was fine. I do not know why it keeps doing this. I am attaching the form.<br />

Can't enter value into first field in Adobe Fillable Form
