Vlookup - 2 criteria

I need to populate column H with a rate based on 2 criteria (Col A and C).
I am using Vlookup for this.
I have something wrong in the formula.
Not certain how to use 2 criteria in Vlookup.

I don’t think I can use TRUE as some facilities have similar names

There;s nothin to match in the Lookup Table on margin so you getting the#N/A error on most lines

Most of the cells in the dates on margin are not dates, they are formatted as General and need to be formatted as a date.

thank you Shums.  I wasn't aware of the date issue on the Margin Sheet.  thanks for fixing that too.
You're Welcome! Pleased to help.
It doesn't matter what date format is used, but it must be a date format not General. Excel stores all dates as integers and all times as decimal fractions. With this system, Excel can add, subtract, or compare dates and times just like any other numbers, and all dates are manipulated by using this system.
