Need excel or access where user can enter data but not see any of the previously entered data

Patients in clinic need to be able to type their name and chief complaint into a spreadsheet or database at a kiosk, but they can't see any data that has been entered previously, for HIPAA reasons (can't see another patient's name or complaint).  All the data needs to be viewable by the registrar so they will know who is in the waiting room.  I've done excel spreadsheet which uses a macro to move the data from the registration spreadsheet into another spreadsheet, which is what I want.  However, I don't know how to make it so the spreadsheet is not accessible by the person standing at the kiosk.

Not sure if I understood the issue.

You may have a data entry form n access. Set filter to say ID=0.
When someone enters data with ID=1000, he sees the data and when accepting the record, it is filtered away.

Other location, can see all the data with proper filtering.

You could do a similar form using Excel with some VBA. Some pdf software will also do this, I think Acrobat Pro and pdfElement

I've tried hiding and unhiding the sheet in excel, but to hide one sheet requires that another sheet be unhidden, I guess so you can see that there is a blank spreadsheet there.  Then the data gets put into the unhidden sheet, not the hidden one.  I couldn't see how to "force" it to go to the hidden sheet.
Just thought of a potential issue with any system you choose.

User arrives at kiosk and fills in information but doesn't submit. Next user arrives at kiosk and can see previous user's details.

Maybe get round it by having auto submit after a period of non activity but then you have the issue of user giving up and leaving rather than not submitting and registrar still thinks user is in waiting room.
Having used Google forms, I have now discovered that Office 365 also has a similar function. It is not as elegant or functional as Google forms but if you already have Office 365 in your organisation it will be available and will not require a link to third party websites.
