Tandberg Tape Drive gone crazy

Hello, I have this Tandberg tape drive where all the lights are flashing. I tried to put a cleaning tape in but I can't get it in. I turned it off and unplugged it 3 times but still all the lights go on flashing when I turn it back on, I don't know what to do, please help. Thanks!

Consult your user's manual.  If you don't have it supply us with the model number so we can Google it.

Flashing lights as you describe are generally faults (in many computer hardware) but without the specific model number we can't go further.

From the chart it looks like "Boot Block Mode e" - I don't know what that means!

 The tape was written without a valid end-of-data mark, which often occurs if you power-down the tape drive while the tape drive was
writing. The tape drive will perform a format recovery, which involves reading the data to determine where the end of data is located. This may take as long as 2 to 3 hours.

I took apart the device, there is a whole tape stuck around the inside spinner, it was probably caught. I tried the tape drive again, but it's catching the tape every time now, I guess it's kaput :(

normally, such an incident does not render it useless..
I think they mean the take-up reel.

Oh well you've still got a good clam-shell case and PSU so you can fit just about any other HH LTO in it although bear in mind that some drives get their power via the SAS connector and some get it via a molex plug. I went to Maplin four times until I eventually picked the right converter to power an HP drive where an IBM one came out of.

There is a plastic strip with a buckle on the end that comes out of the take-up spool then goes around the tape guide path to the front where the buckle engages and disengages with the leader pin in the cartridge. When you pull a tangled tape out that leader could end up anywhere and has to be re-positioned correctly.

I can't find a picture of an LTO leader to show you but if you look at http://support.thesmithlife.com/DLT/DLTDrive-ReattachLeader.html which is a DLT leader you will get the idea. The mushroom circled in blue in the bottom picture doesn't exist with LTO, there is a buckle in place of that but the principle is the same,
The backup tape was replaced with a new one, the old one was thrown away, thanks!
andyalder suggested a faulty tape inside unit which proved to be the problem.
