What is the correct process to follow to create a .CSV file containing the first names, last names, usernames, & passwords of users to import into Server 2016 Active Directory?

Please provide me with a URL resource or guide that describes what other columns need to have which information (such as AD password) and what other information needs to be included within this spreadsheet and in which order this information should be listed.
I'm looking to add on to this existing .CSV file with any additional information and then get the steps in running the import of this information to import this information into Server 2016 Active Directory to create these user accounts.
