PS3 Assassin's Creed IV: Black Flag Issues

I'm officially at my wit's end here.  I have a customer who is a Veteran, and the only game he wants to play is Assassin's Creed IV: Black Flag on PS3.  He brought in his PS3 because the game freezes almost every time at the LOADING screen when the bar gets all the way across.  Here is a list of what we have attempted to repair this issue.  Oh, and yes, he has been able to play this game before.

HIS PS3 SLIM
1.  Disassembled and clean Laser LENS.
2.  Cleaned Disc.
3.  Repaired HDD.
4.  Backed up data, installed used 250GB HDD, installed PS3 Software 4.81, and restored data.
5.  Deleted update file to Assassin's Creed IV: Black Flag and redownloaded.
6.  Deleted saved game data
7.  Used 2nd Assassin's Creed IV: Black Flag disc.

So, in an attempt to figure this out because I told him it's either the disc or his system, he brought in another Assassin's Creed IV: Black Flag Disc for PS3 and a used PS3 Slim that he purchased at GameStop.  Like I said, he REALLY likes this game. So...

NEW PS3 SLIM (NEW to him)
1.  Transferred data using ethernet cable from OLD PS3 to NEW PS3.
2.  Formatted system and created new user.
3.  Kept system from hooking up to internet to download update.
4.  Updated game software with no saved game data or user account.

I will stick with the 'NEW' one for now.  Thank you for your response MASQ.  Here's the current setup.

Currently formatted running 4.81 OS.
No account is signed in besides generic.  Not activated to the PSN.
Game is updated to 1.04.

I don't know how to get to the UPlay your are referring to since Assassin's Creed IV: Black Flag does not get past loading.  Is UPlay located somewhere else that I am misunderstanding?

Argh!  Yes, thinking about the Steam version I think - you can't get to the rebuild option unless you can launch!  Going via the PS' XMB doesn't give you anything to help either.

You can reset again though

Boot the PS3 into Safe Mode
- Restore File System and Rebuild Database
- Restore default settings
- Restart

Even when it's working perfectly AC4 takes an age to load the game first time from a fresh install. Keep it offline and logged out.
I did what you requested and it still sticks at the Loading screen.  This did not take a long time for it to happen though, and I see the update is still currently on the PS3.  So, I am deleting the Saved Game Data (6430MB) and opening the game again (OFFLINE AND LOGGED OUT).  I am gone for the day from work, but will look at it more tomorrow.  Definitely up for more suggestions! :)  Thank you for your help so far.
I have deleted the saved game data and had the PS3 redownload the game from the disc without the internet hooked up.  After 20 minutes, the game data was loaded, but the system still froze at the loading screen after the bar is all the way across.  I restarted the PS3 and tried again.  It takes about 15 seconds to load the game, and then freezes after the bar is all the way across at loading.

The only similar thing I can find is where Ubisoft games need the update source to be online not local.  On installation from the disk if you're prompted for updates choose no, then launch the installed game online and then allow the updates to be installed when prompted - Probably not a lot of help if AC4 isn't launching anyway but short of raising a ticket with them I'm afraid I'm out of ideas, you seem to have covered all the other bases by having two separate installation disk sets and two PS3's so this does sound like a config issue.

Thanks for the update, strange though if it is the drive that both Slims were affected.
I believe that the system was suffering issues with the DVD drive, but purchasing the downloadable version was a solve, so we will go with that.
