Select box php and mysql and autopopulate input fields on form

I have table in database "customer"
and this columns:
id, name, father_name, surname, adress
I want, in select box will be name (father_name) surname, and when I select that I want to autopopulate text input filed "adress". So, I need help with showing value in input field "adress". I use this to make drop down select box:

Thank you Chris for reply. I tryed but no success again. Db connect is ok for sure, because in select box show options from database, but when click on one of that options adress input no changes.. stay empty always. I am not sure for jquery library.... in head of file i put link to jquery.min.js....
Success... load another jQuery :) now it works :)

I have one question more about that. Now that works when I loaded jQuery 3.2.1, but template not showing well when loaded this jQuery. This is template jQuery. When I replace with this, changing on select not work, exactly like before.

Not really sure what your problem is - the code you've provided is jQuery version 1.2.6 which is over 9 years old, so you shouldn't really be using it anymore.

Don't understand what you mean by the template not showing well!
There is example, how it looks like.

And there is jNice.js and new jQuery.js

So that works without jNice.js but not looking good. When i put jNice.js changing of selection not work

OK. Well the jNice plugin is nearly 10 years old and doesn't look to be supported any more, so I would recommend you use a more up-to-date plugin to style your dropdowns. There are plenty out there. Here's a link to one called niceSelect:

I tried to make new html with bootstrap, now jquery not too old, but again same problem :(
If you're using jQuery and Bootstrap then everything should work fine.
