Prime + is not a model number.

Goto Settings -> About this phone and post back the model no and Android OS version.

Goto Settings -> "Backup and Reset" and browse to the lowest option "Reset to factory setting" and you will be asked for your password for Google account.

To perform a 'hard' reset press and hold volume up, the Power button and the Home button.

When the Samsung logo appears release the Power button, when the Android logo appears release the other buttons and you should be presented with the 'hard' reset/recovery menu.

You can then use the volume key to move up/down the menu to whichever option you want and press the Power button to select the required option.
