sql server 2012

hi,
iam having one sp every ten min once the sp will run in scheduled. i have one doubt in that. iam having trigger table in that trigger table any updation and insertion happens pkid will store in trigger table. by joining trigger pkid and my original table i'll get the  changed record.  by checking this i'll update the changed record in one table  (loln_lnmistranprocess)  if not that record i'll insert  (loln_lnmistranprocess)

For Eg: in that table already some cases are processes and marked as record processed as Y . my doubt is some old record processed  y  anything again changed in trigger ( 15 records are tere 5th record got again changed it processed in loln_lnmistranprocess this table how can i identified)

how can i identify in between records changed?

i'll attach my sp
