You'll have to explain more about your data - what it is, how you're getting it (from an import, or entered by a user).

To create a form in Access, you generally create a Table, and then create a Form based on that Table. At it's simplest, you'd have a single Table, and a single Form. Rarely are database applications that simple, however, so that's why we need to better understand your data to help you.

1. First create table with the fields mention. Set their data type.

2. Create form based on that table.

...
If you need more help explain more to understand what you actually want to have.

Hi WS,

That's almost exactly what I said more than 18 hours earlier. Was there some reason you repeated it?
