i want to develop a form in ms access . anybody please help me it very small form

field will be in the form are as follows

GX NO. CG/001/N - AUTOMATIC CHANGEABLE AFTER EACH
DATE - CHANGEABLE
DOO- DATE I WILL DECIDE
SUR NAME- FROM TABLE
INITIAL-FROM TABLE
RANK-STATIONARY FOR EACH PERSON
P. NO.-UNIQUE NO
THERE WILL BE SOME CASUALTY e.g. LEAVE, MOVEMENT, PROMOTION,AMENDMENT
i donot have the idea how to make this form from the so that i can receive the date from table. ple help me.

You'll have to explain more about your data - what it is, how you're getting it (from an import, or entered by a user).

To create a form in Access, you generally create a Table, and then create a Form based on that Table. At it's simplest, you'd have a single Table, and a single Form. Rarely are database applications that simple, however, so that's why we need to better understand your data to help you.
