Go to the cell phone company store for your model . Provide them with identifying information which they will be able to use to find you to their database. They should be able to help you .

If you go to the website of your mobile provider there is usually a "PUK" link.
You will have to login to get your PUK code.
You may need the SIM card number. (Use a magnifying glass on the SIM)

When you get your PUK, just enter the code when the phone requests it.

GENERAL INFO

PUK = Personal Unlocking Key (usually 8 digits).
PUK code is a second security code for your SIM card
It relates to the SIM card - not the phone

If you enter the wrong 4 digit PIN of the SIM too many times,
you will need your long PUK to unlock your SIM.
(The PUK will allow you to set a new PIN for the SIM).

Repeated incorrect attempts (usuallly 10) in entering a PUK, will totally lock the SIM card.
You would then have to get a new SIM card.
