Yes a femtocell is an add on. Think of it as a mini tower. So it is only helpful wheb within its range. And even that isn't a total guarantee.

VoLTE (AT&T calls it HD Voice) can be the cause of issues at times, so turning it off may help.

Also check with your carrier to see if there are network issues in the area.

Signal Strength/Reception is the biggest and main factor that affects the phone receiving a call.  The phone and the phone cell towers in the area will combine to determine the best option.  There's no point in having a phone with great antenna and reception if the chosen network has poor coverage.

Could owner be on phone or internet - NO, If the user is connected over WiFi on the Internet it WILL NOT have an affect on phone reception
Could owner's phone go to voicemail more often because pre paid plan versus contract plan - NO, this should not be a factor
Does this have to do with type of phone - YES, in that different models have better antennas for better signal detection and connection.

Check the networks that cover your area .. check what bands they support and finally look what bands your phone supports

If you're in a rural area and coverage is weak but perhaps you have good Internet/Broadband .. then it might be worth considering some tricks like redirecting the mobile to a VOIP number while you are at home in a WiFi zone

Finally, it might be possible to increase the number of rings before the phone goes to Voicemail .. check with your mobile network provider to see if this is a setting you can customise .. perhaps having a longer period before Voicemail will make it more likely the phone detects the call.

Prepaid plan always have lower priority to connect to the Internet or the connection is restricted to 3G. In such case, there is less chance to drop the call.

If it is a 3G phone, connected to the Internet via 3G, there is less chance to drop call.

AT&T will have the best coverage on average in the United States.

Besides, if phone call is important, it might be better if you restrict your phone to use 3G only or make sure that your phone and your carrier support VoLTE.

That's a very open ended question. In terms of networks, AT&T and Verizon are the biggest, and most likely to provide the best coverage. So it comes down to what region of the US you're looking in.

Reception is an obvious reason that shouldn't be overlooked. Network congestion could be another. Femtocells can help with this situation when at home for example (AT&T calls their offering Microcell, Verizon sells Samsung Network Extenders). Another possible cause could be that the phone just isn't working well anymore, where either software resets or upgrades could be the fixes.
