HTML form - PDF form

I have hard time to google HTML form that have CSS style look like ms excel. Actually, I have PDF form with have a lot of box like those common medical or auto insurance form that usually ask for applicant info. Like address and ssn and etc. at the bottom is usually signature part.

Now. I want to write HTML and CSS code on HTML webpage and use it to replace the PDF. Because we have over 50 PDF so  I really hope to see some sample work and get the project start.

If you know any, please share with me.

File save as
b)
Alert message:
Word will now convert your PDF to an editable word document ...
Press ok
c)
Alt f a (File save as)
Change file type:
from:
Word Document (*.docx)
to:
Web Page ( *.html, *.html)
d)
File name:
RequiredHTML
e)
Press Save
The file is saved in RequiredHTML.html
and related files saved in "RequiredHTML_files" folder.
>> RequiredHTML >> Change the name based on the requirement.
