How do I display radio buttons in a plsql dynamic content

Hi,

I am using a plsql dynamic content in an apex form to display the values
and want to insert the values through g_f01.

This is a multi record block, and I want 5 radio buttons for each row.
upon the users input I should be able to update the records through plsql.

like below code

FOR i IN 1 .. apex_application.g_f01.count LOOP
