Windows Server 2012 R2 - Cluster disk resources time-out and causing RHS.EXE to get terminated & restarted

Our Cluster has 3 Windows Server 2012 R2 (No Service Pack) nodes running on HP ProLiant BL460c G9 Blade servers; Node A & B are in a Location configured with a Shared Storage with a dozen of LUNs presented to each of them wherein Node C is in another Location with Different Storage with similar LUN Setup. Off late on Node A & B, we have a peculiar Problem of Cluster disk resources time-out and causing RHS.EXE to get terminated & restarted - repeating almost every day. Such RHS.exe failure reports 1146& 1230 events in System Log. Active Node is C;  This happens on B; at times on A;

3 Times on B Node & 1 time on A node, this failure even resulted in STOP Errors: 0X9E BugCheck: Critical,964,19123,0x000E,OS,,73: Operating System failure (Windows bug check, STOP: 0x0000009E (0xFFFFE800A0C86900, 0x00000000000004B0, 0x0000000000000005, 0x0000000000000000))

How to identify the root cause of this failure and to address this at the earliest.
