SQL Server as a Service

Are there any companies you recommend for Microsoft SQL Server as a Service (who offer database hosting for a monthly fee, instead of needing to acquire licenses)?

Well, this depends on your budget first of all ...

Almost any web hosting provider offers free MySQL database as a part of the package (http://top11hosting.com/).

But you may find many providers offering MS SQL:

My immediate thought would be  Microsoft Azure https://docs.microsoft.com/en-us/azure/sql-database/

Though I have never used them, have heard good things about Rackspace : https://www.rackspace.com/en-au/managed-hosting/database-services/microsoft-sql

And google https://cloud.google.com/sql-server/ to consider along side Amazon https://aws.amazon.com/rds/sqlserver/

It also depends on "why" -  For example, if you plan to do the management of your DB then Amazon EC2, but if you want them to manage, then RDS

And you do need good comms bandwidth....
