I wear a lot of hats...

"The solutions and answers provided on Experts Exchange have been extremely helpful to me over the last few years. I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

Check the account settings in Outlook:
File > Account > Exchange Account \ Change > More Settings > Security. see if the box next to "Always prompt for logon credentials" is selected

It could also be the Windows credential manager. Control Panel -> Credential Manager -> Windows Credentials -> Remove the Outlook credentials. Open outlook, input known credentials, and click Remember password.
This happened to me with a shared mailbox as well.

Are the O365 accounts in-cloud only or synchronized from AD via AAD Connect?  I suppose you are not using AD FS for SSO?

If synched, make sure the latest version of AAD Connect is installed and configured for Seamless SSO (SSSO) with password hash sync (my preference).

With that said, both solutions will need to add the following URLs to the local intranet zone of IE or other browsers.

To get this working you will need to ensure you have AAD Connect version 1.1.654.0 (at a minimum).  Then, follow the SSSO Quick Start Guide to either enable SSSO PTA or PHS.
