Sounds like some type of homework / assignment.  You'll need to make a reasonable try at it yourself first.

Being an homework, I would prefer to point you how to solve this, instead of giving you the solution.
Give a look on the MONTH and YEAR functions.
To get the total of a field, use the SUM function but you'll need to GROUP BY other fields to obtain the desired values.
