You can set the page size on the Report Design Tools >> Page Setup >> Page Setup dialog:

Page SEtup
If you don't have a specific size, your printer driver is at fault. Try selecting a different printer in the "Use a specific printer" setting.

Maybe my question should read 'how does one create a report using ledger/tabloid paper if there is no such printer/driver installed?
Can one create a custom report based on a theoretical printer?

You can build whatever you want within the limitations of Access.  It allows a max width of 22 inches

You could use the Microsoft PDF printer and adjust that to use Letter format:

Letter option
When printing, the user can select another printer, but your report design will remain.

 Just to be clear, the answer to this is basically no. A printer driver must be installed.   However the printer may not physically exist.    Many driver installs now though check for a printer, so sometimes it can be tough to get a driver installed if you don't physically have the printer.    But you can do it with different drivers. As gustav pointed out, using a PDF driver is a good one to try for.

Then as far as the page setup, you would need to select the "ledger" paper size.   If the driver didn't have that size, most (all as far as I know) allow you to specify a custom paper size.    But you should find a ledger size on just about every driver out there because it is a standard paper size.

But Access reports are always worked with in the context of a defined printer.   You cannot design an Access report without a printer being defined on the station.
