Here you go, exactly what you need...

vSphere PowerCLI command to get VMs list with snapshot name and descriptions etc.. and export to CSV if you wish..

First install PowerCli

Second for a easy use of powershell script I recomend you use a Powershell editor, I user PowerGui

Then just copy past the script to a new PowerGui session and click run.

Mine is Window 2012R2 server. How to install the PowerCLI on the server ?
