Strange problem with Switch Stack. Switch drops to provisioned.

Hello Experts,

We have a site with 2960x switches, all multi switch are stacks, some have 4, some 3, mostly 2.

Randomly, the stacks loses a switch and nothing in the logs, just when you run "show switch detail" one of the switches will no longer be "ready", but instead will be "provisioned" So far physically rebooting these switches fixes the problem.

This is happening with all stacks.

What is IOS version? Did it happen to start recently? Were there recent configuration changes on switches (or IOS upgrade, maybe)?

Just in case, for start issue disable auto install feature (if not already disabled on switches) by issuing "no vstack" on all switches.

The software version is;
Cisco IOS Software, C2960X Software (C2960X-UNIVERSALK9-M), Version 15.2(2)E5, RELEASE SOFTWARE (fc2)

We bought these switches 3 months back, the issues has started from past 4 weeks,

I have disabled auto install feature.
I did not run into that bug (although I never used 15.2(2)E5). I guess could be Cisco bug, but, if it started after 2 months of uptime it is very unlikely.
Are all switches in stacks use exactly the same baseline? If not make all switches to run the same IOS version.

You can try to upgrade at least one stack to some other IOS version to see how that stack is behave after upgrade.

I have upgraded two stacks with the following code but still same issue.
This problem occurs all of a sudden.
