requesting leave

i need to write a letter to request for a leave for 5 as my parents are visiting me,  i will be available to work from home if needed.

need a subject line too

can someone rephrase the above in to formal letter.

"I request leave for (x amount of time) as my parents are visiting me.  I will be available to work from home if needed."

I know the question has been closed, but I thought I would offer my opinion and advice about your question.

If your contract of employment or your country's employment laws state that you are entitled to a certain number of days leave every year, DO NOT volunteer to work from home, or to answer questions, or to provide any work related assistance in any way UNLESS you are asking for your days off at very short notice and you are therefore looking for a favour from management.

In countries where employees are legally entitled to a certain number of days of annual leave, or where a contract of employment specifies this, your time off work should be uninterrupted by anything to do with work.  Most companies require that employees must give enough advanced notice for their leave request to be granted, and most will not allow too many people in the same departments off at the same time.  If you are not able to offer the amount of advanced notice your employee requires, then you might have to offer an inducement to management such as doing some work from home, but if you are providing sufficient notice you should not be having to offer this service and management should already have had a contingency plan in place to cover your absence.

I accept that not all countries are the same, and that some small companies find it hard to cover peoples' absences, but if you cannot book your slot for vacation once a year (paid or unpaid - doesn't matter) it amounts to slave labour with you being the indentured servant.

Just my opinion.
