SharePoint Online form development - where to start

All,

I am creating a SharePoint Online request form. I have the layout of what is needed for this form.  Not having any experience on how to do this I need direction on where to start.  I believe that I already have Office 365 installed I just need first steps.
I have read on this and it seems that I will need the following:
1. Add the site (somewhat straight forward).
2. Create the Excel List with the fields for the data (somewhat straight forward).
3. Create the form - This I do not have any idea how to do or where to start.

There is alot of information out there, but I need to complete this task rather quickly.

If I could be provided with Youtube videos or other documents that get to the step by step that would be greatly appreciated.

Again, any help is greatly appreciated.

Create a custom list in SharePoint and you'll automatically get an add/edit form.

Settings -> Add an App -> Custom List

It will look something like this in a modern site page;

So I managed to drop the fields into the page, the fields are in columnar or tabular, is there any way to move the fields around manually so the page looks more aesthetic?
You can change the order of the columns by dragging and dropping in a modern page, or List -> List Settings -> Column Ordering in a classic page.

That's all you can do out-of-the-box in terms of moving the fields around manually. Let us know if you're looking for something more!
