Sometimes when I open a message in Outlook, it shows that there's an attachment (1), but when I open the message, the attachment is not there (2). Where is the attachment hiding?

Can you give me a screen print of the email, it could have been stubbed by an archiving application, so if you run something like Mimecast you'll need to check that.

The only time I see this is for a signature or vCard file (agree with the above). In-line pictures are not attachments. Did you check this?
