Sometimes when I open a message in Outlook, it shows that there's an attachment (1), but when I open the message, the attachment is not there (2). Where is the attachment hiding?

Can you give me a screen print of the email, it could have been stubbed by an archiving application, so if you run something like Mimecast you'll need to check that.

it could be because there is an image in the signature. review the email that this is happening to and review the signature and see if it's an image. the image could be pics or a pic that looks like a text signature but it's actually an image.

The only time I see this is for a signature or vCard file (agree with the above).  In-line pictures are not attachments. Did you check this?
