Issues With Mapping User Certificates

I have an issue with my certificate authority not mapping certificates to user accounts when requested. I have a two tier PKI setup with an offline rootca. When I request a user certificate the certificates are not automatically mapping to the user accounts. I have been able to request them and manually map the x509 to the user mappings. However, I cant seem to get this certificate to work for authentication.

what template are your base for the template you've created? How is it not mapping to user account? Is the certificate not added to the AD user object in AD or is it not issued to any devices?
