Wordpress form loads with endless loop.
Wordpress form loads with endless loop. After I modified a Wordpress form, it just goes through an endless loop when loading and does not allow input. I don't remember what changes I made to the form that started the problem. The URL for the form is: https://www.barnwellmd.com/intake-questionnaire/ When I preview the form in Wordpress, it works properly. I don't know how to extract the form from the Wordpress system to allow you all to work on it. Any help will be appreciated.

In what way did you modify it? Did you do it through the dashboard of Wordpress? If so, did you use a plugin to create the form?
Looks like the problem is you've referenced some link in your form which is going into an infinite loop on your site.
So looping at the Webserver level, not WordPress.
So... remove your NGINX front end + start with Apache only first. Also remove all site relative links from your form.
Then start adding links back in one at a time till your find the problem link.
If you have access to your Apache + NGINX logs, tail the logs + visit your page + you'll see the problem instantly.

2) Add define( 'SAVEQUERIES', true ); to your wp-config.php file.
3) Visit the page + let the page hang, as it's doing now.
4) Eventually your page may timeout + if it does, you may be able to look at debug bar -> HTTP requests to see problem.

The apache and NGINX logs are blank. What would you charge to look at it?
Normally Apache + NGINX logs are always blank, because logging is setup incorrectly... er... configured a level to low to be useful...
You must first configure your entire LAMP Stack - Apache + NGINX (best to just remove NGINX till you resolve problem) + database + PHP + since you're running WordPress set WP_DEBUG to true.

If you see looping messages, then your WP debug log file is catching the problem.
If not, then check your Apache log files for looping.

With further testing, I discovered that Multiple Columns for Gravity Forms works. The problem occurs when a drop down field is selected for one of the fields in the multiple columns and Enable add options is selected for the drop down field.
