Find your original card where you pulled off the SIM card, the pincode along with the PUK code should be  there. Also if it's not, it's in the self service part online of the telco official customer portal (if they have one).

The PUK code isn't on the Sim card, the PUK code is held by the network operator and they use this to unlock your phone, I.E if you put your pin in too many times they can provide this number to unblock the phone.

It's a security measure, it's not the number on the SIM card i'm afraid.
