Could be a number of things

1. the windows version you are running is not updated & thus the certificate that signed this DLL is not installed in your trusted CA
2. there is a newer version of Outlook that you should upgrade to

in short it is most likely an update missing somewhere

it will trust the root and yes it shouldn't show up again

anyway if you don't want to click "trust all" you can start by fully updating your windows and Outlook first

the certificate that is singing the code of this add-in will be added to the trusted certificates of your computer so that other code signed by the same Certificate will be auomatically trusted
