how to put header with some thing like  "Design Document"

Insert tab ―> Header & Footer group ―> Header options ―> Edit Header and this will open the Header & Footer Tools tab.

From here you can enter whatever header & footer text you wish.

with the Header & Footer Tools tab still open, again, click on the Insert tab ―> Text group ―> Quick Parts ―> Field and select the Date and Time category and select the fields you want to appear in the footer.

 i do not see TExt group i see TExt Bix next to PageNumer

You do indeed have the Text group.  A group is a subdivision of the Ribbon.  If you look at your image, at the bottom of each group is the name, so you have 8 groups: Pages, Tables, Illustrations, Links, Comments, Header & Footer, Text, Symbols.  So the instruction were referring to the before last group Text in which there is a button Quick Parts.  Once you click on it you'll then have Field as an option.

Word -> Innsert -> Quick Parts -> Field ...
Can you post the document?

How did you create the page numbers?
Did you use Insert -> Page Number (in the Header & Footer group) -> Bottom of Page and then made your selection of the desired format?
Word -> Insert -> Page Number
