Hmm, are you certain they were sent? Is it possible user was working offline or clicked the send button twice?
check outlook rules or transport rules or group memberships.

What it has to with rules?
Well I provide support for a company.

I belong to the "support@company.com" distribution group.
So if I sent emails to support@company.com I get them into my mailbox (on the inbox)

Another way is the Outlook rules:
https://support.office.com/en-us/article/manage-email-messages-by-using-rules-c24f5dea-9465-4df4-ad17-a50704d66c59

You can set rules to get duplicates.

As well as the transport rules
https://support.office.com/en-gb/article/inbox-rules-in-outlook-web-app-edea3d17-00c9-434b-b9b7-26ee8d9f5622

You'd need to find out where the problem is.
