I am in the process of taking on some new bsuiness customers for my IT support business. In the past I have gone into the customers businesses and gathered all the informaton I need about their systems (username, passwords, Line of business applications etc etc) whilst I am there.<br />

To streamline this process and to ensure I get all the information I need at first contact, I want to create a form for them to complete in advance.<br />

I wondered if any of you guys would be willing to give me some sugestions as to what information I should ask for. If anyone is happy to provide me with the questions you ask your customers then I would be very greatful.<br />
