software development new and change requirement template format

Dear Experts

I am looking for new and change requirement template as we are in process of implementing ERP we recently went live but operational users and stake holders are asking for new requirement and change requirement would like to implement the policy where requester fills the form which should have columns like Purpose,  and manager to approve cost and efforts, thanks in advance.

Most organizations end up with something that is customized to best fit their culture and specific organization needs, but here's a good document (in my opinion) that should help get you started.  There are a number of pages at the beginning that talk about the "why" of a change request form, and then the last couple of pages show a specific example.  It seems to capture a lot of the information we requested at the last shop I worked (I'm semi-retired now) so thought it might be useful for you.  I'm attaching the document, and the reference link is:

What are you using for your software development? (I'm assuming you develop software internally)
Whatever the tool (TFS, JIRA, etc.) I'd recommend creating a new project or just a new ticket type for "Feature Request" such that users can create their requests directly in the tool you are using for managing development, using the same format that the developers use for User Stories.
Those tickets will need to be triaged by a member of the team or ideally the product manager/owner for the product and validated.
From there they can be converted into proper user stories and added to the backlog for development.

Typically most organizations I've seen do this with one form, for either a new feature, or a change to an existing one.
