how to create form in Sharepoint

we have a departmental Share point site. i have a database in access. how to enter the data using the form in Share point. the format could be excel.

Take a look at MS Forms. The form has various question style options, multiple choice or text entry.

The form results are then submitted to a spreadsheet.

Should be able to access this link through an Office 365 subscription. Basically works like survey monkey; you set the form up with questions, when completed the link is shared to those that need it and the submission sends results back to the author.
