Removing overlayed images from a photo.
you need an image with layers which .png/.jpg don't have

That is a child of one of our church friends. We wanted to give her a surprise framed photo of the child she has not seen in 15 years. But it looks like he put the heart in the wrong place. He speaks no English so no way I can ask him for the photo before he added the overlays.

What language does he speak? Based on attire and surroundings, I would guess he is Hispanic (I grew up in South America and looks similar to what I saw during that time) and speaks Spanish. If so, you can ask him, "tienes este photo pero el original?" If spoken, it would sound like:
"Tyeh-ness ess-teh foh-toh pear-oh ell oh-ree-hee-nahl ?"

Yes Spanish but with this being a surprise for his mother he has failed to communicate when anyone in the USA since he does not know them.
It might be worth writing a letter explaining who you are and what you're trying to do, then just ask someone to translate it for you. There are many people who know both English and Spanish and would be happy to lend a few minutes of their time for a good cause. There are a lot of missionaries and missionary kids who scatter around the US after retirement (or after kids reach college-age), so it might be worth asking whether or not there are any such folks within your church who can translate written English to written Spanish.
Ideally, you'd be looking for someone who was from the boy's specific country, since there are various dialects of Spanish, but even if you can't find someone with that specific dialect, it's usually close enough to be intelligible (same way that people in the US can understand most of British English and vice-versa).

We have tried our own Mexican pastor and 6+ FB friends of the mother and no success.
Do you have the original photo with the "sticker" overlays but without the grid (which looks sort of like a screenshot from a cropping operation in Lightroom)? There's a VERY slim chance that doing the Photoshop "painting-in" with some generic content and then running a reverse image search could bring up the original (without heart/smiley face stickers) photo if it's indexed by Google anywhere.
