Found the answer:<br />

<a href="https://www.apple.com/watchos/feature-availability/#branded-ecg" rel="ugc">watchOS Feature Availability</a> displays if the region has the feature (ECG in my case). &nbsp;So I click on it and the region where I am at is not available,<br />
<br />
Yet, the issue is that the watch was purchase in the USA, so it should be available right? &nbsp;<br />
<br />
Still searching why ECG doesn't work in a USA purchased device.

Apple Watch series 5 options of ‘Setup ECG’ and ‘Setup Irregular Rhythm notifications’ says  “apps not available on your region” when trying to setup.  

My account has always been setup in United States and in Apple Watcha setup it displays it:
