I have installed PowerShell version 6, but my servers still show as version 4.0 when I issue $PSVersionTable.
What else I have to do to make that I have version 6?

It's a side-by-side installation, PS6 is not an upgrade of version 1-5. You can use both PS 1-5 ("Windows") with 6.x ("Core") at the same time.
You should find a link the start menu or via search, or start pwsh.exe (should be in the path), or using the full path:
C:\Program Files\PowerShell\6\pwsh.exe

On a side note: consider upgrading PS 4 to v5.1.(this will be a real upgrade!)
As the Windows and Core versions work side by side, the Core (PS 6) has script-breaking changes, and it does not have all cmdlets available in PS 4/5.

Thank you for the quick reply,
How do I run ISE?

You don't, at least not in PS 6.
The new official development environment for PS is Visual Studio Code:
