Is it possible to collaborate on one file in SharePoint. The problem is that when we have several users editing the same file it seems to create a second copy. Is there something we are doing wrong? Is there a file permission issue? Any advice would be appreciated.

Its Office 365. So SharePoint Online.
So your saying that the modification can only take place online and the users need to make sure they are accessing the document via the browser and not the desktop app?

For Excel that is the case. If anyone edits using the Desktop app they lock the file.
Users are then prompted to save a copy, and that might explain your duplicate files.

so this is not the case in Word? if so is this a glitch Microsoft is working on?

Excel is an outlier as it can only be co-edited in the browser, not in the desktop client.
What version of SharePoint are you using? What sort of file is it?
