Is there a way to create an input form and report that scrolls / displays from left to right rather than top to bottom?

I want to show and input to

Pay App 1  |  Pay App 2 | Pay App 3

rather than


Pay App 1

Pay App 2

Pay App 3

where Pay Apps 1-3 are rows in a table

Sure,
Select the table in the navigation pane
Select: Create-->Forms-->More Forms-->Multiple Items

Just note here that your fields have repeating names:
Pay App 1
Pay App 2
Pay App 3
...This is a sign that your design may not be properly "Normalized"...

Let's be clear, ...are these "Fields" (Field Names) or values in a field?
...or something else...?
