Let's be clear, ...are these "Fields" (Field Names) or values in a field?
...or something else...?

Then continue working with Dale on this, ...I was presuming that these were individual fields...

Select the table in the navigation pane
Select: Create-->Forms-->More Forms-->Multiple Items
Just note here that your fields have repeating names:
Pay App 1
Pay App 2
Pay App 3
...This is a sign that your design may not be properly "Normalized"...
